Diabetes Self-Management Training

The Diabetes Self-Management Training Program at Catskill Regional Medical Center (CRMC) provides comprehensive, individualized programs designed to help people with diabetes attain optimal health. We provide support and improve quality of care for people with diabetes throughout Sullivan County and the surrounding areas.

An American Diabetes Association-recognized program with a focus on self management, we teach patients how to take control of their diabetes and provide them with the skills necessary to manage their diabetes on a daily basis. A highly qualified and dedicated staff of professionals works in conjunction with your primary care provider to give you the best in collaborative care.

The CRMC Diabetes Self-Management Training staff consists of Certified Diabetes Educators including, a Registered Dietitian, Registered Nurse, and an Exercise Physiologist with expert input from a Board Certified Endocrinologist.

Whether you are newly diagnosed or have had diabetes for years, we design a management plan just for you. Highlights of our program include:

  • Group classes for self-management education.
  • Management of diabetes during pregnancy.
  • Insulin injection technique and management.
  • Individualized nutrition assessment and counseling for diabetes.
  • Insulin pump training.
  • Continuous Glucose Monitoring.
  • Ongoing diabetes education including the latest methods of treatment and refresher classes.

CRMC's Diabetes Self-Management Training staff recognizes that knowledge is the key to managing your diabetes and strive to provide information to you in a warm, fun, and interesting manner. We provide support and improve quality of care for people with diabetes throughout Sullivan County and the surrounding areas.

We recognize the importance of working in conjunction with your primary care provider and thus, we require a referral for our services. We accept most insurance including Medicare and Medicaid.
