Catskill Regional Medical Center’s Geriatric Assessment Program is a comprehensive medical examination completed by a Board-certified physician who specializes in geriatrics, as well as a registered nurse and dietitian. The Geriatric Assessment not only evaluates the disease process and progression, but also the person as a whole.
Your medical history will be obtained from your primary care physician. The information will be used by our staff to assure that we have all the necessary information to provide you and your physician with suggestions on how you can remain healthy and active.
The Assessment Program can assist with any of the following concerns:
• Memory Loss
• Confusion
• Incontinence
• Multiple Falls
• Nutritional Problems
• Depression
• Weakness
• No Social Support System
• Frequent Hospital Admissions
• Loss of Independence
The Geriatric Assessment manager will assess the patient’s eligibility for needed services such as:
• Personal Care Services
• Medicaid
• Durable Medical Equipment
• Adult Day Health Care
• Personal Emergency Alert System
Assess the patient’s symptoms
• Memory Loss
• Disorientation
• Confusion
• Poor Concentration
Establish a diagnosis
Provide the patient with recommendations to assist them, as well as their family members and caregivers, in coping with Alzheimer’s or other dementia and plan for both present and future needs.
Day one
Time Span: Four Hours
• Medical History
• Falls Assessment
• Dementia and Depression Evaluation
• Diet and Medication Assessment
Day Two
Time Span: One Hour
• Comprehensive Physical Examination by Geriatrician
The patient, caregiver and physician will receive a comprehensive geriatric assessment report with findings and suggestions on treatment and needed services.
